Establishing with a Primary Care Physician is an important step in maintaining and preventing health complications.   Your decision to join the Dr. Hill Family is the best decision you can make for you and your loved ones.  To ensure that you get the care you deserve, follow the steps below for our "to get established" process.  

Step 1

First contact your insurance company to verify that Dr. Hill is a participating provider for your particular plan.  Let them know that you wish to have him as your primary care physician.

Step 2

The next step in establishing with us is to provide us with some basic information.  Download, print, and fill out the "Patient Packet" below and forward it to our office by faxing, mailing, or bringing it by. This step will save time and ensure that we assist you very quickly. 
 
Step 3
Once you have completed this information and sent it our way, we will contact you to set up your "to get established" appointment.   If you would like to make an appointment request, you may do so by clicking here

Step 4 
Again, make sure you have notified your insurance company, prior to your first visit, of your decision to have Dr. Hill as your primary care physician.  This will ensure that proper payment is rendered on your behalf.  Remember to bring your drivers license, insurance cards, and all medications in their bottles. Arrive to your appointment at least 15 minutes early so we can verify all information, scan a digital copy of your cards, and collect your copays (if applicable).  If you are going to be late, please call us as soon as possible so we can make adequate schedule changes.
